This is a more comprehensive questionnaire.  In the Fitness Assessment section you’ll note it asks for “Body-Fat Measurement Setting:”  It’s important to record the mechanical setting of the body-fat testing equipment you used.  This information will help you remain consistent with your clients.

You’ll also see the last few questions help qualify your client’s “level of intent.”  We recommend you tear this page out of the Journal (or photocopy) and keep in a client file folder for accurate record keeping.
