The Client Profile and Follow-up Notes is to track your client’s Progress.  You’ll note the space for an ID #.  Many fitness professionals assign their clients a number or simply use a membership number for reference.  

 

Again, you’ll see the space for client information.  This will refresh your memory if you have numerous clients.  The sheet has spaces for specific body measurements every four (4) weeks.  Beneath that is a record for each weekly appointment.  We’ve provided space for crucial data.
